Parents Annual Information – Grade 9
In preparation for the 2018-2019 school year, there are a number of essential forms and pieces of information that require your immediate attention. Many of these forms are regulatory in nature and are legally required by the Ministry of Education for Southridge to operate. All of the forms require your electronic signature.
Please take the time to read the posted information thoroughly and completely. You can print copies for your records if you wish.
The link to the “Parent Letter for Duke of Edinburgh Award” is to be reviewed by all parents and the “Duke of Edinburgh Registration Form” must be printed, completed and returned to the school should your child wish to participate in the Duke of Edinburgh program in Grade 9. Copies of the form will be available at the school should you encounter difficulties printing the form.
Please note that this process needs to be repeated for each child enrolled at Southridge as information and consents are required for each child individually. You may use the grade grouping sections in the menu at the top of the page to navigate to the forms required for the next child, if necessary.
